Root Cause Analysis & Problem-Solving

Overview

Root Cause Analysis (RCA) is a set of methods, tools and techniques used for identifying the source of problems, faults, failures, defects and undesired outcomes for processes. It is widely used in manufacturing, services and public sector organisations.

 

This 1-day highly interactive training course introduces delegates to different problem-solving methods, tools and techniques, which can be applied throughout a series of highly interactive and practical exercises putting the theory into practice.

Collaborating in teams, delegates will use techniques such as the 3Cs (Concern, Cause, Correct/ Countermeasure, 8Ds (8 Disciplines, introduced by Ford), 5 Whys, Fishbone diagram, PDCA (the cycle of continuous improvement) and localisation to solve a range of product and process-related problems, and in each case present findings back to the group.

 

This will prepare delegates for typical workplace RCA practices, including problem identification, RCA tool selection and report writing.

Learning Objectives

On completion of this training course, delegates will be able to:

 

  • Participate as contributors in problem-solving teams

  • Use the appropriate tools and techniques to identify, and document the root causes of problems in production and administrative processes, related systems, and product or service design.

  • Identify, record, evaluate, group, and prioritise solutions to prevent problems from recurring

  • Validate their findings and make recommendations to their colleagues and senior management

  • Engage senior management in improving the process/ system by removing the root causes of identified failures

  • Perform risk assessment, pilot and implement the selected solutions

  • Standardise to maintain the gains from problem-solving

  • Monitor and control the improved process/ system to establish that it is stable and delivers the anticipated outcomes/ benefits

  • Hand over the improved process/ system to BAU (day-to-day) operations

  • Place the improved process/ system under Change Control, to prevent unauthorised changes that may destabilise processes that are currently ‘under control’

Prerequisites
